If you would like to open a new service ticket please complete the following form and we will contact you to schedule your service call with a technician as soon as possible. Please provide as much detail as possible including model/serial number of unit if possible and if available, symptoms, and if unit is currently functioning, or any other concerns.
Please note: Clients with active FHSi Service & Maintenance Agreements always receive first priority when our team members are scheduling service tickets.
